Flu Vaccine

Friday, November 4th!

Dear Parents,

The Bryan County Health Department will be at Silo Elementary Gym to administer  Flu Vaccinations on Friday, November 4, 2011 starting at 9am.  If you would like for your child to receive this flu vaccination, please fill out the form, sign it,  and have your child return it to the school.  The vaccine is available to PreK/Headstart students through 12th grade.    It is highly recommended that parents be present for the vaccination and parents are required if child must be restrained.   If you have any questions you may contact  the school nurse at 580-924-7000 ext 244.
