The following forms must be completed and signed BEFORE athletes can participate in activities.

• OSSAA Physical Form – completed by a physician and signed by parent/guardian
• Sudden Cardiac Arrest Form – completed and signed by parent/guardian
• Concussion Fact Sheet – for information only
• Concussion Acknowledgement Form – completed and signed by parent/guardian
• Biological Sex Affidavit Under 18– completed and signed by parent/guardian
